CoreQ Survey

CoreQ was developed by a team, including Nicholas Castle, Ph.D., the American Health Care Association/National Center for Assisted Living (AHCA/NCAL), and providers with input from customer satisfaction vendors and residents.

It is a short three/four question survey that measures the major drivers of patient and family satisfaction. It's designed to be added to any survey tool the care provider is already using. It can also be administered on its own. This tool has been created for all Skilled Nursing and Assisted Living care providers. There are different versions of the survey for each care type.

Skilled Nursing

    4. How would you rate how well your discharge needs were met?
  2. Long-Stay Resident Satisfaction

    3. How would you rate the care you receive?
  3. Long-Stay Family Satisfaction
    1. In recommending this facility to your friends and family, how would you rate it overall?
    2. Overall, how would you rate the staff?
    3. How would you rate the care your family member received?

Assisted Living

  1. Resident Satisfaction

  2. Family Satisfaction

Every question of the CoreQ survey has a five point rating scale:

  1. Poor
  2. Average
  3. Good
  4. Very Good
  5. Excellent

The CoreQ survey is designed to create a single measure somewhere between 0 and 1. It is reported as a percentage. The percentage is calculated by dividing the number of positive responses by the total responses. A response is considered positive when the respondent's average response for each question is Good, Very Good, or Excellent. Pinnacle is one of the providers of the CoreQ survey for SNFs and ALs. In December of 2017, the Center for Medicare Services (CMS) announced that it was considering adding CoreQ to the SNF Quality Reporting Program (QRP). They are only considering adding the Short-Stay Discharge Satisfaction survey.
